Mcp Ratchet Clinical Charting is a Model Context Protocol (MCP) server that enables the Claude AI assistant to directly document patient visits into Electronic Medical Records (EMRs), aiming to reduce the administrative burden for home health nurses. It provides structured tools for searching patients, creating visit notes with vitals, and retrieving patient histories, all integrated within the Claude Desktop environment. Currently available in mock mode for development and testing, full production integration with PointCare EMR is forthcoming.
